O Requerimento tem de ser acompanhado com os documentos constantes em "Que documentos são necessários?".
For all types of exam:
- Identification document
- Passport photo (one)
- Medical certificate for seafarers (issued by a doctor on the list of doctors recognized by the Directorate-General for Health)
- Maritime note (cover sheet and records of shipments and landings, duly stamped and initialed)
- Proof of sea service
- Basic security certificate
- Qualification certificate for the operation of lifeboats and rescue boats
- Qualification certificate for the control of firefighting operations
- Qualification certificate to provide first aid on board vessels
- Certificate for resp. for healthcare on board vessels (management functions)
- Qualification certificate for the exercise of specific protection functions
- Appropriate recycling course certificate (when applicable)
- Previous certificate of competence (when applicable).
For each type of exam:
Deck Section - OCQN (II / 1)
- Certificate of the 1st cycle of the Pilotage Course or equivalent
- Training log book (when applicable)
- General Operator Certificate in GMDSS
- Training certificate for the operation of ECDIS (operational level)
- Arpa Certificate in Simulator
- Certificate of training in resource management on the bridge - BRM (operational level).
Deck Section - Cte and Immediate (II / 2)
- Certificate of the 2nd cycle of the Pilotage Course or equivalent
- Certificate of qualification for ship security officer
- Training certificate for the operation of ECDIS (management level)
- Arpa Certificate in Simulator
- Certificate of training in resource management on the bridge - BRM (management level) (when applicable).
Deck Section - OCQN limited to coastal voyages (II / 3)
- Certificate of approved education and training course
- Training certificate for the operation of ECDIS (operational level) (when applicable)
- Arpa Certificate in Simulator (when applicable)
- Operator certificate in the national GMDSS A1 and A2.
Deck Section - Cte limited to coastal voyages (II / 3)
- Certificate of training in resource management on the bridge - BRM (operational level)
- Training certificate for the operation of ECDIS (operational level) (when applicable)
- Arpa Certificate in Simulator (when applicable)
- GMDSS Restricted Operator Certificate.
Machinery and electrotechnical section - OMCQ (III / 1) and Electrotechnical officer (III / 6)
- Certificate of the 1st cycle of the Machinery and Electrotechnical Engineering Course or equivalent
- Training log book (when applicable)
- Training certificate in maintenance and repair of electrical, electronic and control systems (when applicable)
- Training certificate in operation management and safety of electric power installations (above 1000 V) (when applicable)
- Certificate of training in resource management in the machine room - ERM (operational level).
Machinery and electrical engineering section - Chief engineer and second engineer (III / 2, III / 3)
- Certificate of the 2nd cycle of the Maritime Engineering Course or equivalent
- Training certificate in maintenance and repair of electrical, electronic and control systems (when applicable)
- Training certificate in operation management and safety of electric power installations (above 1000 V) (when applicable)
- Certificate of training in resource management in the machine room - ERM (management level) (when applicable).
Machinery and electrical engineering section - OMCQ (III / 1), Chief engineer and second engineer (III / 2, III / 3) limited to coastal voyages
- Certificate of approved education and training course
- Certificate of training in resource management in the machine room - ERM (operational level) (when applicable).
Each exam: € 108.50. The value of the certificate is € 50.50.
The urgent service fee is € 50.50.
