The Application must be accompanied with the documents contained in "What documents are needed?".

• Certificate of competence
• Qualification certificates
• Maritime note (cover sheet and records of shipments and landings, duly stamped and initialed)
• Declaration by the master of the ship attesting the competence of the seafarer to carry out the duties to be performed
• Declaration proving the non-existence of seafarers with the desired qualifications
• Medical certificate for seafarers.

The fee for urgent service is €118,40.
